I want the divets on Madison Street in Capitol Hill to be fixed

There are a ton of weird divets and bumps on Madison Street in Capitol Hill near the crossing of 14th Ave (see the map below). They really suck to drive (and walk) over and it'd be really awesome if they were repaired.

Here's a form on the seattle.gov Web site where you can submit a street maintenance request form (aka report a pothole): http://www.seattle.gov/transportation/potholereport.htm. You can also call their pothole and street repair hotline at: 206-684-ROAD (7623).

Their goal is to fix potholes within 48 hrs. (2 business days) of receiving a report. According to their Web site, their current success rate is 91%.
